双侧人工全髋关节Ⅰ期置换治疗髋关节疾患18例

摘 要 目的：探讨Ⅰ期双侧全髋关节置换术(SBTHA)治疗双侧髋关节疾患的可行性及疗效。方法：自2002年4月至2006年12月，18例双侧髋关节病变患者采用Ⅰ期THA 治疗。年龄45~72岁，平均54.6岁。术前Harris评分平均42.3分(33～59分)。结果：手术时间3.5～5.1h，平均4.4h，术中出血600～1100ml，平均780ml，术中输血600～1000ml，平均720ml。术后X线平片示假体位置良好。18例术后平均随访21个月，关节功能Harris评分平均82分(78～85分)。结论：应用Ⅰ期双侧THA治疗双侧髋关节疾患，安全且近期疗效满意，可以缩短住院时间，减轻患者痛苦及经济负担，值得推广应用。临床应注意慎重选择病例，严格掌握手术适应证，并做好充分术前准备。

ONE-STAGE BILATERAL TOTAL HIP ARTHROPLASTY FOR

THE TREATMENT OF HIP DESEASE

Abstract Objective：To evaluate the feasibility and therapeutic effect of one-stage bilateral total hip arthroplasty (SBTHA) in treating hip disease. Methods：Eighteen cases of hip disease were treated by SBTHA between April 2002 and July 2006.Their average age was 54.6(ranged from 45 to 72).The mean Harris score was 42.3 preoperatively (ranged from 33 to 62).Results：The average time of operation was 4.4h(3.5～5.1h).The average blood loss was 780ml(600～1100m1)and blood transfusion was 720ml(800～1000m1).The X-ray after operation showed that the prostheses were in good places. Eighteen patients were followed up for 7 to 59 months(averaged 21 months);The harris score of hip function was 82 on average (78~85). Conclusion:SBTHA is safe and satisfactory for the treatment of hip diseainge. It can shorten hospitalization, relieve patient's distress ,and alleviate their economic burden. It's worth recommending for the bilateral hip diseainge. However selectiny of cases should be carefully, operation indications should strictly be considered, and preoperative preparations sufficiently made.
